• After hospitalisation, to support recovery at home.

• To support end of life palliative care if you or a loved one wishes to pass at home.

Our nurses bring clinical wound care right to your home, but it's more than just bandages and ointments. We’re here to create a calm, safe, and supportive environment that helps you heal, both physically and emotionally.

• Catheter Care

Managing a catheter can seem like a lot, but we’re here to make it easier. Our nurses support you in keeping everything clean, comfortable, and simple, helping you maintain hygiene and reduce the risk of infections. Together, we’ll make catheter care a reassuring part of your daily routine so you can focus on feeling your best.

• Continence Reviews, Assessments & Reports

Being comfortable and confident is so important. Our continence assessments help us understand your needs and create a plan to improve or manage any bladder or bowel control issues. This isn’t just about your physical health, it’s about giving you a sense of dignity and independence, which can boost your emotional and social well-being, too.

• End of Life Care

If your loved on is living with a life limiting illness, you'll face important decisions. We'll discuss your loved one’s desired level of support, honouring their wishes, preferences, and spiritual or cultural needs. As your loved one’s care needs change, we'll work closely with you and your family, ensuring open communication at every step, so this chapter is as comfortable and peaceful as possible.

Are nursing services covered under aged care or NDIS?

Clinical nursing services will be fully covered under the new Support At Home program that starts 1 July 2025.

Nursing services may be covered under government funded aged care, this would be outlined in your Home Care package.

You can learn more about Home Care packages and the new Support at Home program and services here.

If you access the NDIS, your plan will list the services and supports that can be funded. NDIS supports are designed to provide support for living with a disability.

There is a defined list of NDIS supports that are funded. If you are admitted to hospital, the health system is responsible for your hospital and post-acute care service.
